Job Description:

Schedule: Full Time

Shift: Monday – Friday, 8:30am – 5pm, Rotating weekends and call.

Location: We have need for PTs to perform home visits in one West Volusia County. Travel to other areas as needed.

• Other duties as assigned. 
• Develops patient-specific care plans with realistic, measurable goals in consultation with patients, families, and physicians that Considers the physical, cultural, psychosocial, spiritual, age-specific, and educational needs of the patient.
• Reassesses patients at appropriate intervals, documents progress, and determines the effectiveness of therapy.
• Observes and records patient activities and findings, reporting any changes or deviations in the plan of care to the physician.
• Instructs patients and caregivers on the use of assistive devices and establishes home programs.

Knowledge, Skills, and Abilities:
• Ability to delegate tasks to appropriate personnel as indicated by skill level and professional standing
• Home Care Regulations and Third-Party Reimbursement as it impacts care delivery
• Demonstrated knowledge of physical therapy theory, modalities, treatments, and skills
• Basic computer skills and knowledge of Microsoft Office.
• Must be able to read, write, and speak conversational English.

Education:
• Doctorate [Preferred]

Field of Study:
• physical therapy

Work Experience:
• 1+ year relevant clinical physical therapy experience [Required]
• Recent, relevant experience in a Medicare-certified home health agency as a care-manager [Preferred]

Additional Information:
• Education requirements for this position are determined by the licensing or certification standards set forth by the relevant certifying body. Applicants must possess the level of education mandated for eligibility to obtain and maintain the required license or certification.

• Driver's License (DL) [Preferred]
• Basic Life Support - CPR Cert (BLS) [Required]
• Certified for Oasis Specialist-Clinical (COSC) [Preferred]

Pay Range:

$34.71 - $64.55
