Works independently and with direction of the Nurse Manager and Nurse Director to coordinate the multifaceted aspects of the Orthopaedic Service Line with focus on the patient¿s experience, care, and outcomes.
Other information:
Minimum 3 - 5 years clinical experience required.
Clinical expertise caring for Orthopedic/Neurosurgical patient populations preferred.
Orthopaedic Nursing Certification (ONC) or obtain within 1st year
Experience in:
Quality improvement and / or utilization management preferred.
Multiple software applications including, Excel and data base management preferred
Case management, quality/utilization review, or outcomes management preferred
Clinical expertise caring for Orthopedic/Neurosurgical patient populations preferred
Demonstrated team leader. Must be flexible and self-motivated
Organization, strong interpersonal and written communication skills required
Basic computer skills in Windows and Word required
Responsibilities:
Patient Education: Patient, family, significant other and community education programs: Serves as an Educational Leader for the Patient Population in area of specialty. May create, participate, and/ or coordinate education across the care continuum that would address patient educational needs relating to specific patient populations; Patient and community education programs. (Pre-op education classes, daily patient rounds, follow up phone calls and health fair, etc.)
Staff Education: Serves as an Educational Leader for the staff for the Patient Population in area of specialty. May create, participate, and/ or coordinate education across the care continuum that would address staff educational needs relating to specific patient populations; provides unit based educational programs related to best-practice and AAOS, NAON standards. Educates staff on new equipment, products and Orthopaedic related procedures, etc. May create, participate, and/ or coordinate education across the care continuum that would address staff educational needs relating to specific patient populations. Supports the education and mentoring of new team members.
Performance improvement: Process and quality performance improvement projects. Disease specific performance improvement measures for TKR, THR, Spine, and all orthopedic patients. Analysis of data and follows plan do check act process for improving measures.
Data Collection/Quality indicators/ measurement: Collects data for Patient Population in area of specialty. Develops, implements, and participates in Quality Improvement processes. Works with members of the healthcare team in activities designed to improve patient care outcomes (daily audits on unit-based process improvements i.e. EPCs, neurovascular checks, foley d/c, etc., Work closely with the infection prevention nurses to report infection rates
Multidisciplinary Team Collaboration: Collection, collaboration, analysis, and dissemination of quality and outcomes data. Conducts monthly Orthopaedic service line meeting by planning, conducting and disseminating of minutes with multidisciplinary team. Agenda includes Orthopaedic volumes, Orthopaedic dispositions, team concerns, performance improvement measures, patient¿s satisfaction, team concerns, equipment, etc.
Communication: Communicates and collaborates daily with the entire Orthopaedic service line team member including nursing, case management, inpatient rehabilitation services preadmission testing, same day surgery, surgical services, home health, post-acute facilities, and other departments ad hoc.
Patient Experience: Demonstrates a commitment to customer service with patients, visitors, and other customers as measured through patient satisfaction and department surveys. Reviews patient experience data on a regular basis, responds to complaints in a timely manner utilizing established communication processes. Implements actions to improve (Follow up phone calls, patient satisfaction committee participation, daily patient rounds).
Management Assistance: Shares accountability with manager for unit/department/organizational goals achievement. Provides managerial support as required.
Role Model/ Resource: Creates and maintains a safe and therapeutic environment for patients, self, and team members. Acts as a role model for others by communicating responsively, respectfully, and assertively with patients, families, physicians and peers as well as other members of the healthcare team. Clinical resource to staff. Clinical expert caring for Orthopedic/Neurosurgical patient population.; Patient and community education programs (Identify and communicate with PAT, physician offices, health outcome coordinator, home care liaison, and other members of the healthcare team).
Patient Population Power Plans in area of specialty: Monitors, creates, educates, and updates power plans in area of specialty in coordination with the team specifically the nursing Informatics, Electronical medical record team and physicians. Works to coordinate and follow service line best practices by reviewing standards, attending conferences and collaborating with surgeons.
Meetings: Attends meetings related to patient population in area of specialty as well as unit based
Joint Commission: collaborates with the hospital Joint Commission accreditation specialist. Participates in disease specific joint commission surveys. Prepares information/agenda, collects data, disseminates information, prepares the team, monitors data and participates in joint commission routine updates throughout the accreditation period.
