This position is within the Home Based Primary Care (HBPC) program of Montana VA Health Care System (MTVAHCS). HBPC is a home care program that provides comprehensive, interdisciplinary, primary care in the home of Veterans with complex medical, social, and behavioral conditions for whom routine clinic-based care is not effective. The duties and responsibilities of the incumbent are carried out from the Montana VA assigned stations (may include incumbents own home). The occupational therapist treatment site is in the homes of Veterans. S/he is responsible to attend and contribute to weekly Interdisciplinary Team Care Conferences at the local station (Community Based Outpatient Clinic (CBOC) or main facility). The program serves Veterans living within 100-minute radius from assigned station. This position reports to the Home Based Primary Care program manager. The Home Based Primary Care (HBPC) OT at this level practices independently and is responsible for comprehensive assessment of functional and occupational roles using standardized and non-standardized evaluation tools. OT's select and provide direct occupational therapy interventions and procedures with varying degrees of complexity to enhance safety, wellness, and performance in AOL, instrumental IAOL, education, work, play, leisure, and social participation. OTs demonstrate a full professional understanding of the theories of treatment in occupational therapy and the proper application through the use of activity analysis, behavioral intervention, frame of references, and therapeutic procedures. The HBPC OT may also be a subject matter expert and may have certifications relating to home evaluations and Home Based Primary Care such as Certified Aging in Place Specialist (CAPS), Certified Low Vision Therapist (CL VT), Assistive Technology Practitioner (ATP) and other clinical specialists as deemed appropriate. PRINCIPLE DUTIES AND RESPONSIBILITIES Screening/Assessment- The occupational therapist will develop an in-home assessment of the veteran in collaboration with the interdisciplinary treatment team, the veteran, family members and other caregivers whenever possible. The goal of the in home assessment is to highlight the veteran's strengths, limitations, and internal/external supports and service needs in order to optimize the veteran's functional status and safely maintain the veteran in his/her home. A treatment plan based on this assessment will be completed within 30 calendar days of veteran's admission into the HBPC program. Treatment Planning/Goal Setting - The occupational therapist will independently provide a comprehensive in-home assessment, inclusive of veteran's occupational performance, performance skills, client and environmental factors to compile an overall plan of care in collaboration with the interdisciplinary treatment team, the veteran, family members and other caregivers, whenever possible. Incumbent possesses the advance practice skills to develop an intervention plan based on evaluation findings, occupational therapy theory, and evidence-based practice and collaboratively implements the plan. Ongoing assessments and updating treatment plans are done when necessary or on a quarterly basis given the changing needs as health status changes with the veteran. Occupational therapist addresses the unique needs of the homebound, frail, disabling/chronic and/or dying veteran. Work Schedule: Monday through Friday 8:00 to 4:30 with the possibility of compressed/alternative work schedule for highly qualified candidates. Financial Disclosure Report: Not required
