You Will Be Responsible For:
Using evidence-based practice (EBP), works with medical and program directors on maximizing the patient care experience, expectations and outcomes through standardization of clinical patient readiness/optimization of care pathways. Participates in the development of Orthopedic Practice Standards including pre-op, post-op, transitional orders/instructions and post-acute care (PAC) protocols. Optimizes outcomes while monitoring and reducing cost-per-patient episode.
Meets with patients/families, develops plans of care, aligns patientsâ™ needs and goals with post-acute care treatment planning. Communicates goals with physician and post-acute care providers. Incorporates whole person care with CREATION Health model promoting quality care and cost-effective outcomes that enhance physical, psychosocial and vocational health of individuals.
Monitors clinical progress for first ninety days after transition to post-acute environment. Communicates delays in care, functional status and changes in clinical status to surgeon or primary care provider (PCP) and coordinates required follow-up and monitoring. Monitors and documents responses to interventions and client progress utilizing motivational interviewing to facilitate positive behavior change and meet goal-driven outcomes. Monitors PAC facility performance against EBP goals and reports variances to manager weekly.
Assesses clientsâ™/caregiver(s)â™ activation stage, literacy level and self-management capabilities. Identifies and refers client to appropriate interdisciplinary care team member and/or community resources for identified needs in these areas. Incorporates the assessment and referrals into plan of care. Coordinates efforts to improve patient outcomes and reduce readmissions by ensuring MD office visits are completed, monitoring medication reconciliation, following up on outstanding tests and procedures in the PAC environment.
Educates client and family/caregiver(s) on diagnosis, medication, medication reconciliation and nutrition and moves client toward self-management.
What Will You Need:
EDUCATION AND EXPERIENCE REQUIRED:
Minimum of two years of experience in orthopedic nursing
EDUCATION AND EXPERIENCE PREFERRED:
Bachelorâ™s or Masterâ™s degree in nursing
Minimum of two years of hospital or post-acute care coordination experience
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
Current valid State of Florida license as a Registered Nurse
LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED:
Orthopedic nursing certification â“ ONC through the Orthopedic Nurses Certification Board (ONCB)
Accredited Case Manager (ACM), Certified Case Manager (CCM) or RN-BC (Board certified in case management)
Cardiopulmonary Resuscitation (CPR) OR Basic Life Support (BLS)
The Nurse Navigator - Post-Acute Orthopedic Care is responsible for the transition and coordination of care, patient engagement, education, patient satisfaction and clinical excellence post-joint replacement/fracture repair at AdventHealth Orthopedic Institute (AHOI) - Central Florida Region. Provides joint replacement patient education through one-on-one format and patient education/support groups. Facilitates an evidence-based, multi-disciplinary approach to support patients via navigation of the healthcare continuum. Develops and maintains collaborative relationships with hospital, surgeons and post-acute providers. Takes an active role in facilitating communication of patient needs to the physician in an effort to avoid unnecessary delays in care/functional improvement/readmissions. Coordinates efforts to improve patient outcomes by ensuring MD office visits, monitoring medication reconciliation, following up on outstanding tests and procedures in the post-acute-care environment. Works closely with physicians to assist with patient education data collection and research development. Monitors clinical, quality and other key regulatory metrics. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
