JOB TITLE: Care Management Assistant LOCATION: Geisinger Medical Center Hospital DEPARTMENT: Care Coordination WORK SCHEDULE: Days/Evenings WORK TYPE: Full Time JOB SUMMARY: The Care Management Assistant supports the department of Care management and the staff in their day -to-day responsibilities. The Care Management Assistant performs duties and tasks in accordance with performance standards established for the position. Plays a key role in obtaining information about patients with medical and social needs. The Care Management Assistant acts as a resource to assist patients in navigating the health care system. Under the direction of the Care Manager and Social Worker, serves to facilitate linkage to community resources that will enable the patient to obtain optimal health outcomes and transition safely to the next level of care. Directly accountable to the Director or Manager of Care Management. COMPETENCIES AND SKILLS: Demonstrates excellent written and oral communication skills Demonstrates the ability to collaborate with other members of the healthcare team and providers in a positive, professional, progressive and assertive manner Demonstrates the ability to perform multiple tasks simultaneously Demonstrates exceptional ability to retrieve clinical information from medical chart/documentation. Ability to motivate and engage patients and the Health Care Team Ability to openly address and acknowledge observed issues and concerns Attention to detail, and ability to be flexible in performing a variety of tasks Demonstrate ability to complete projects within designated timelines and the ability to prioritize duties is required Demonstrate proficiency in basic computer skills Demonstrates knowledge and regulatory mandates EDUCATION AND/OR EXPERIENCE: High School Diploma required. Minimum of three years’ experience in hospital or health care based work experience preferred (Physician office, clinic, or community based outreach). Ability to work in a fast paced environment, including responsibility for accurate and comprehensive documentation, independent problem solving using pre-established guidelines and stratified work flow. Previous experience and familiarity with medical terminology. Valid PA Driver’s License.
